Wiki Telephone appointment by Psychiatrist

My psychiatrist does phone appointments for some of his patients that are either away at school or out of the area temporarily. Because several of psychiatry codes changed in 2013, I'm not quite sure which codes would be best suited to this situation.

Are they paying out of pocket? If so I would just bill them as if it were being done in the office.

Is there any cases the EKG//ECG should not be attached to chest pain dx???Any reason?
