Wiki Telehealth visit and other services rendered on the same day (eg. X-Ray, Rapid Strep)

If a patient has a telemedicine/telehealth visit and the provider places an order for the patient to come into the clinic the same day for an X-Ray or a Rapid Strep test, should we bill the codes on one claim noting two separate place of services (POS) OR should we bill two separate claims for the services rendered? Any guidance would be greatly appreciated.

I am also looking for information on telemed visits. If a provider determines the patient needs to be seen in the office during the telemed visit can you bill a sick visit in office for the same dos?
