Has anyone tried to bill for Transitional Care Management Serivces, 99495 and 99496 before? My physicians are telling me that they can bill for these but I'm a little confused when reading all the rules and guidelines. It seems to me like a Urologist would not do this. Can anyone give me any input on this? Thanks!!
 
The TCM codes are brand new for 2013, so nobody's billed them out yet, since they're billed following a 30-day time frame post-discharge.

If your urologist was the discharging physician, or the discharged patient is still with a global urology surgical period, your urologist cannot bill.
 
TCM codes

I just read an article in Internal Medicine News and the only exclusion I see will be hospitalists. Since the codes are the being described as paving the way for the Medical Home it would seem that only primary care physicians would be reinversed. We are going to try it since we are internal medicine. Anybody else have anything. I would also like to hear other views.:rolleyes:
