Wiki TCM Codes 99495 & 99496

We are a surgical practice and I am trying to get a handle on TCM (transitional management care) codes. I am not sure if we can really utlize these codes since most of our pt's are post op during the 30 day period after dischare but not all. Can anyone help.

Most surgeons cannot use the TCM codes. That's written right in CPT: "The same individual (provider) should not report TCM services provided in the postoperative period."
