If a patient is discharged and seen IN CLINIC within 2 business days but hasn’t had the outreach phone call (2 day interactive call) after discharge, can the provider use the TCM code since the patient had a full office visit within 2 business-days?
Yes...we did this if the patient was seen within the 2 business days. Look at the definition under the "Interactive Contact." It states the contact may be made via telephone, email or face-to-face. As longs as the elements are documented within your note, you are covered.
