Wiki Sylatron Billing under Medical insurance

One of our docs wants to start a patient on Sylatron for melanoma - It does not have a j code for billing - Has anyone else had any experience billing this drug? I want to know if it should be billed under medical or pharmacy benefit.

I am doing more research...but any input would be appreciated.
