Patient diagnosis: Ocular Cicatrical pemphigiod with symblepharon and cicaticial entropian and trichiasis. And inclusion cyst of carnucle.
Procedure:
The symblepharon was identified and an incision made in the conjunctiva removing the symblepharon and creating a free conjunctival edges in order to suture the amniotic membrane graft. Sharp blunt dissection was taken to bare sclera and all adhesions were removed in order that the conjuntivoplasty and amniotic membrane graft could be accomplished. This was harvested and placed in position and sutured with interrupted 10-0 Vicryl sutures on both the bulbar and tarsal conjunctival edeges. The inclusion cyst was then removed by sharp and blunt dissection without difficulty. Vicryl sutures were used to re-approximate these tissues as well. A bandage soft contact lens was placed and symblepharon ring placed. The culdesac was found to be reformed properly. Ocuflox was instilled and Polysporin placed over the area of the inclusion cyst removal. A pressure patch was placed and the patient was taken to the recovery room in good condition.
We coded: 65780, 68335-59, and 68236 ? but upon review from insurance company, they denied code 68326 as included in 68335. Stating the medical notes does not support the code billed or mod used.
