On the date that a swing bed patient's status changes from inpatient to nursing facility, what CPT code set would the hospitalist use - subsequent hospital?, initial nursing facility?, subsequent nursing facility? I do not believe that this qualifies as a hospital discharge and nursing facility admission because I believe there is a merged record. Any input is greatly appreciated! Any source where I can find (clear) official instructions would be appreciated as well.
