Wiki Surveillance Colonoscopy

Doctor did a follow up surveillance colonoscopy on a Medicare patient but did not provide the patient's DX history from the previous colonoscopy.

The H&P states s/p colonoscopy 2003, no symptoms.

I queried the doctor requesting previous DX history and the reply was "I don't have that HX available".

Any suggestions on how this can be coded? Thanks.
