Wiki Surgeon performs anesthsia

We have a new surgeon on staff that does their own anesthsia in office and in a hospital setting. They claim to have been paid for both services in their previous practice, but we are having trouble getting the anesthsia paid when billed with the surgery. Is there a certain way the claim needs to go out to get full payment?

The anesthesia codes we bill are 64421-59,47 for the office visits and 64450-59,47
Nerve blocks are the main procedures described. Is there anything missing?

Any help is greatly appreciated!!
