Supervising MD for E&M's in facility and office, MDCD Managed Care plan. Appending Modifier SA, as appropriate. Are the MD's paid at the mid level reduced rate? Or should they be paid the MD fee schedule rate?

You would need to review your payer's policy and/or contract for the answer to this. It could be different for each state and each managed care plan, and if your provider is contracted there could even be an exception.
