Can you point me to where I can find supervision requirements for procedures? Catheter changes, bladder instillation, uroplasty....procedures that would be done in the office and can be done by the nurse, would I put the dr that ordered it in the supervising spot or the dr that was actually here? Dr. X ordered it, Dr. Y was in the office when the procedure was done.
 
You can try looking up supervision requirements on the CMS website (mostly all I found was outpt hospital). I work for a medium sized urology group, I always list the provider that is in the office as the supervisor and the doctor that ordered it as the supervisor (Medicare incident to regulations). If no provider is in the office, we usually won't do procedures other than UA's or bladder scans or simple cath changes
