I think you should read the guildlines in the CPT on each of those sections. There are set rules, and we follow them.
Take a look at one of the excision codes in the musc/skel section. You'll see they use the term subcutaneous. If the lipoma is deep below the top two dermal layers and in the sub-q tissue, that is more than likely the section you want.
Don't be thrown off by titles of sections....in the "skin" section there are codes for debriding down into "bone".
