Wiki strep & med refill for bee allergy

We had a patient in for a sore throat. She ended up being diagnosed with Strep and while she was in the moc requested a refill on her medication for her bee allergy.
We are using 034.0 for strep and V15.06 for personal history of allergy to insects. My question is, should I also add V58.83 encounter for therapeutic drug monitoring?

If you use V68.1 Issue of repeat perscriptions, it has to be first listed code only. I would only code 034.0 for strep that is all the MD is treating right?
