The patient was brought to the operating room and placed in the prone position. The area of the lower lumbar region was prepped out and draped in a sterile fashion. Areas of back pain were demarkated by the patient and incorrporated into the planned field. A linear incision was infiltrated with local anesthetic. Incision was made with a 10 blade and carried down to the fascia of the mid lumbar area and a self-retained retractor were placed. Four Tuohy needles were placed under fluoroscopic guidance and the subcutaneous electrodes 2 left and 2 right side,were placed and position confirmed with fluoroscopy corresponding to the patient's deliniated areas of pain. An area was given local anesthetic in the right hip area. Linear incision was then created with a 10 blade and a subcutaneous pocket for the battery was then performed. Subcutaneous tunneling for the wires was then done. Intraoperative stim trialing was performed and found that all areas of pain were well covered with stimulation. Extension splitters were then connected to all wires and connected to the battery. The thoracic wound was closed using 2-0 Vicryl suture for the muscle and the fascia. The patient was then awakened and tested with the stimulator and all electrodes seemed to be working well. A simple coil of the wiring was then performed and tacked down the the fascia and the wire was plugged into the battery. Impedance checking was then performed, showed good impedance on all contacts. The battery was placed into the pocket with the charging side facing outward and extra wiring was placed underneath.
