This gentleman had:
Cystoscopy.
LEFT stent removal.
RIGHT ureteropyeloscopy.
RIGHT laser lithotripsy.
RIGHT stone basketing.
RIGHT stent placement.
Insurance is denying because stent removal bundles. However, because it is on the opposite side, wouldn't that be reimbursable? I don't see why the LEFT stent removal would bundle with the RIGHT stent placement.
...Bladder was inspected. Left stent was identified, engaged with grasping forceps, and extracted without difficulty. A 0.035 guidwire was passed up the right ureter under fluoroscopy and I was able to pass an 8.3-French ureteroscopy without needing to dilate. The scope was advanced into the right collecting system...
...and so on and so forth. The remainder of the report is right side.
