Patient present to clinic for Valstar instillation and catheter change. Upon removing the patient’s Foley Catheter, his chronic indwelling stent was present at the urethral meatus. Patient was transferred to surgery center and under local anesthesia was placed on the operating table.

At this point fluoroscopy demonstrated that the proximal aspect of the stent was still in the ureter. Therefore, the entire area of the genitalia anterior thighs and scrotum was prepped and a wire was placed through the existing stent and advanced into the kidney. The existing stent was removed and a new 6 x 24 Optima stent was placed coaxially over the guidewire. A good curl was noted fluoroscopically in the kidney. Initially the distal aspect of the stent was in the area of the bladder neck. A catheter was used to place the stent in its proper position since the patient needed a catheter change anyway. This was a 18 French catheter, which was left in place.

not sure on cpt or if I can just charge for the catheter change???
 
This reads to me as 50387 antegrade.
But Id ask the physician, did he replace this retrograde 52332 or antegrade?
