Physician Billing;
Can you bill under the physician for split shared services on an admit of an established patient if the physician documents his component of the note properly? Or, am I confusing myself with "incident to" guidelines?
 
As long as the physician sees the patient face to face on the same day and documents his involvement as well as agreement/disagreement with the NP note the yes you bill using the provider number.
 
Shared/split visits are defined by CMS in IOM Publication 100-04, Chapter 12, Section 30.6.1(B) as an E/M service "shared between a physician and a NPP from the same group practice, and the physician provides any face-to-face portion of the E/M encounter with the patient. A substantive portion of the E/M visit involves all or some of the history, exam, or medical decision making key components of an E/M service." If the documentation meets the requirements the visit can be billed under the physician's PIN, as opposed to the NPP's PIN.
