I have an OP note in which the patient had an existing fusion with hardware at L5-S1. The doc went in and did the following:
Explored the fusion at L5-S1
Removed the hardware at L5-S1
Performed arthrodesis (curetting end plates, discectomy) at L4-5
Intervertebral body spacer at L4-5
Placed hardware from L4-S1
I believe the coreect codes for this is:
22633- L4-5
22851- L4-5
22842- Instrumentation from L4-S1
However, the surgeon wishes to also code 22830 (Exploration of fusion) and 22852 (Removal of hardware from previous spinal fusion).
Would y'all please look and tell me how the AAPC thinks this should be coded?
