Wiki spine add-on codes deny as duplicate

Our add-on codes for spine surgery keep getting denied as duplicates. Has anyone had this issue, and if so how were you able to resolve it.
An example:
22612 L2-3
22614 L4
22614 L5 -denied as duplicate,

I always indicate each spine level with each code.

we do the 22614 with # of units rather than separate line items.
 
My Medicare carrier requires us to bill units and our BCBS requires something completely different. I'd verify with the carrier(s).
