Wiki SNF medicare phy fee

If I have a SNF in our facility (we are a CAH) but a visiting specialist is seeing that pt from our Rural health clinic (which is hospital based), can I bill Medicare for the physician fee and do I have to use a modifier? Both the RHC and CAH share the same tax id. (I know all the Outpt services ie labs and xrays are lumped under the skilled encounter)
Any help would be greatly appreciated.
