Can some help me with this. I am not sure as to how to code. I use 17110 for the wart do I use same code for acrochordon?

1. Acrochordon (L91.8)
Destroyed on neck, inner thigh
2. Viral wart (B07.9)
Treated as below on neck

Clinical impression: skin tags (1 likely viral wart on anterior neck)
Indication: therapeutic
Location(s): neck, R medial thigh

Verbal consent obtained detailing R/B/A of procedure: yes

Procedure:
Skin was prepped with alcohol. Lesion(s) removed or destroyed using iris scissor. 1 lesion on anterior neck treated w/ cryo w/ 3 freeze/thaw cycles prior to removal w/ scissors due to being suspected wart.

Total number of lesions: 6

Skin tag removal

History of Present Illness

Has a few skin tags around her neck and one on her R inner thigh she would like removed.
 
Skin tag removal is usually not covered by insurance unless the lesion is causing some sort of pain, bleeding, itching, etc. and that symptom has been reported on the claim. Check your LCD for benign lesion removal. In our jurisdiction, L91.8 (keloid scar uncertain) by itself will not be covered, unless an additional (symptom) code from group 3 is reported with it. B07.9 is a covered Dx under 17110. 17110 includes up to 14 lesions, and can only be billed once on a claim. Hope this helps!
