Does anyone know how to bill for this? The patient had Mohs microsurgery the day before by a different physician, then the next day my physician did an I&D lt. middle finger squamous cell carcinoma excision site, skin and subq tissue with a full thickness skin graft from the lt. volar forearm to lt. middle finger wound, 1.5 x 3cm. We submitted it as 15240 and 11042-59 but the debridement was denied. Any help would be appreciated.

The debridement is considered to be a part of the graft. The modifier will not work unless there is a separate site for the debridement.
 
Mitchellde I have a question: Doesn't a graft need to be coded with a surgical preparation code? In Cpc Exam papers those figures are given along with and the general guideline is to add them too. Is in it? That too a Ca site like this?
There is a sort of confusion in my mind always as regards this. Can you clear up my lasting doubt about this.
