Hi,
I'm a little confused with the documentation requirements with 31090. We had a physician use a 0 degree endoscope to remove polyps and diseased tissue from the maxillary sinus and performed a total ethmoidectomy. He only mentions that he is able to view the sphenoid sinus and it is clear (nothing removed from that sinus area).
My instinct is telling me to code 31267 and 31255 but the dr wants to submit 31090. When I read the ENT desk reference it states for 31090 the physician enters 3 or more sinues o remove diseased contents using multiple approaches. The physician uses multiple approaches to the sinus including intraoral, intranasal, skin and/or bicoronal incisions. Incisions are repaired, etc. The dr used a scope and currettes and biting forceps to remove the diseased tissue and he only removed it from 2 of the 3 sinuses he entered....
