Wiki Simple mastectomy - male patient

I need some feedback on coding simple mastectomy for a male patient. Dr. states he removes all tissue to muscle, never mentions gynecomastia. The path report states gynecomastia once received. I once found info that states can only use this for male if breast CA is diagnosed, but cannot find this info.
