This may be a strange question, but I am having trouble with this in our facility. I have been getting H&P's that have no physician signature at the end, electronic or otherwise. What I am getting is at the very top before the H&P begins :

John Doe, MD, Physician, Signed, Hospital Service, H&P, Date of Service 00/00/0000, time stamp


Is this acceptable for a signed H&P by the physician. The nurses are saying that this is sufficient and acceptable for an electronic signature.
Are they correct. I have just never seen an H&P signed this way.
And the very last page states: "Thank you for allowing (name of facility) to participate in the care of this patient."
I am just getting the impression that this is something the the facility may have printed from their EHR.
