Our facility just started using ABNs for laboratory test. I would like to apply modifier GA to the encounter but wonder if it should be attached to the lab test (s) or to the office visit.

Agreed. The modifier should go on the lab test with the reason it may be denied. It sounds like your practice is going to get an ABN on every lab patient they see? Your practice management may want to give that some more thought. Getting an ABN on every patient tends to throw up some red flags as they are to be used when there is a good chance or thought that the test will be denied. If the patient is coming in for annual lab draws to check existing conditions (cholesterol, diabetes, etc) it really isn't a good idea. Just my thoughts and others may have different ideas about this. It's worth the research :)
