Can you code a distal clavicle excision seperately when the orthopod did a 23430 ( open biceps tenodesis), 29823 ( shoulder debridement), and a 29826 ( subacromion decompression/acromioplasty)...I feel like I am over coding..so right now I have the following codes

23430
29824-59
29823-59
29826

I do not see that there are any bundling issues in the 4 codes you have listed. Which is surprising, considering all of the bundling issues I normally see with shoulder surgeries! Unless there is a code you haven't listed, I don't see why you have the 59 modifiers on 29824 and 29823? They do not bundle, but if they did you wouldn't be able to bill them for the same shoulder. I hope that helps!

The -59 modifiers would be because they are being done arthroscopically, correct? I agree, there are no bundling issues I can find, but yes, if your code choices are correct (primary procedure being an open procedure, and the remaining being through the arthroscope) then you should be in the clear.
