My mind is making this more complicated than it really is - or so I'm telling myself that this case is not as difficult as I'm trying to make it be. 
Here's my question: Can 29819(removal LB) be billed with 29822(debridement)? Or do I just bill for removal of LB (29819) since it's the higher code?
Doctor did a biceps tentomy, removed the periosteum from underneath the acromion and removed loose bodies from the axillary pouch.
Patient had complete tears of the supraspinatus & infraspinatus, but these were deemed unrepairable, therefore, they were not touched.
