Wiki shoulder scope converted to open procedure

My doc did a shoulder arthroscopy, type 1 SLAP debridement of the superior labrum, multiple forein bodies and loose body remove and open revision rotator cuff repair.

He wants to bill it as 29827 and 23412-59.

I am not sure this is correct coding.

Please advise.

Proc:

Standard posterior arthroscopic portal was made in the right glenohumeral joint. a working anterior arthroscopic portal was made using needle localization. Probe was inserted. There was a type 1 SLAP tear and this was debrided to a stable rim. Anterior posterior labrum and biceps tendon were intact, as were the articular surfaces. There was a small area of wear of the glenoid and that was debrided using a shaver performing a glenoid chondroplasty. There were multiple loose sutures, foreign bodies in the shoulder and these were removed under direct visualization. The arthroscope was then removed and inserted into the subacromial space. There was noted to be a large retracted cuff trear. At this point, it was decided to convert to an open procedure. Therefore, the arthroscopic instruments were removed. The anterior and posterior
portals were closed with 3-0 nylon sutures. The lateral portal was enlarged approximately 2 inches. Full-thickness flaps were elevated off the deltoid. The deep Gelpi retractor was placed through a split in the deltoid and extensive synovectomy was performed removing scarred synovium. Multiple retraction sutures were placed on the anterior half of the rotator cuff tear and then the posterior half was carefully elevated out. Multiple margin conversion sutures, 5 were placed bringing the anterior and posterior halves together. One BioCompote anchor was placed posteriorly. The sutures were passed through the posterior half of the tear and tied down with excellent result and then 2 additional SwiveLock anchors were placed, one in the middle of the tear and one anteriorly with excellent 100% repair of the tear on tension. Therefore, the tails of the suture were cut. Shoulder was washed out with normal saline.........

23412 (open RC) + 29823 (labrum + glenoid chondroplasty). You can 't report 29827 + 23412 together. CMS instructs you to use open procedure when converted. Some try to add 22 mod but it's very difficult to get extra $$ plus it slows payment down.
