A Fukuda retractor was placed to retract the humeral head and a pitchfork retractor was placed on the anterior aspect of the glenoid. This allowed exposure of the anterior glenoid. The malunion of the bony Bankart fragment was removed using the osteotome. A bur was used to prepare the anterior margin of the glenoid to a nice flat surface to accommodate the allograft.
The Arthrex distal plafond prep kit was utilized to prepare the allograft. The appropriate template was used to size the allograft. The allograft was fashioned accordingly. I place initially with temporary K-wire fixation. Once the graft had been appropriately contoured and sized. a definitive fixation was achieved. This was done using the Arthrex screws. Two 34 mm screws were utilized. Excellent compression at the graft site was achieved. There was good restoration of the articular surface of the glenoid.
Attention was now directed to capsulolabral repair. A 2.9 mm Suture Tak was placed along the margin of the allograft. This was double-loaded anchor. The two sutures were then used to repair the capsulolabral complex medially along with the anterior margin of the allograft.
Lateral repair was performed using two Arthrex corkscrew anchors. a 4.5 mm corkscrew anchor was utilized. These were double-loaded. The capsule was repaired in a T-plasty like fashion. The inferior leaf was brought up first and superior was brought over. The same sutures were then used to repair the subscapularis. The subscapularis repair as repair was reinforced using multiple # 2 nonabsorbable sutures placed in a figure of eight fashion.
