Need cpt for sharp excision of cyst. The doctor marked destruction of a lesion, when he really did sharp removal.

Does the note give dimensions with margins, mention going into the dermis, and/or placing stitches to repair the resulting defect? If so, you may want to look at CPT 11400-11446.

If the note refers to removing the raised portion only (so that the defect is essentially even with the surrounding tissue) without a closure, use CPT 11300-11313.
