Wiki Sequencing Modifiers

I am having a brain freeze this morning regarding placement of modifiers. Can somebody give me the sequencing rule for modifiers? Ex 11721-GA-59 or 59-GA?

Modifier

You would use 59 first, then the GA

I believe the rule of thumb is sequence the modifiers that have direct influence of reimbursement rates prior to any others.
