what condition should be in current header or assessment for sepsis and severe sepsis, not clear when to code sepsis and when not to code in HCC ?

Sepsis must be caused by some problem such as blood infection, organ failure, infection from catheters, or toxic in body. Using Sepsis usually needs 2 dx codes or combo codes from proper documentation. Provider also should list type of infection strep, E Coli, anaerobes, Etc. the dx blocks of A40-A50 describe types of sepsis. Complications listed can be found J95, T80-T88 O86 if pregnant, plus labs and symptoms can tell provider if patient has sepsis. Also B00 and B97 types of infections too. And urosepsis is not valid dx code too. Dx Z22 list if pt carrying bacteria but no symptoms or at risk.
