Wiki semantics of "1 or more sessions" or "1 or more stages"

Many CPT descriptors contain the qualifiers "1 or more sessions" or "1 or more stages". Does this imply the service is only billed once, regardless of the number of subsequent encounters, or is it billed with each subsequent service? And if so, is modifier 58 applicable with each subsequent stage?
