Thoughts on billing the patient a straight fee as self pay without going through insurance?

Even if it is denied, you should bill it to the carrier so it is counted toward the deductible.

Get a form drafted up stating that the patient agrees to pay out of pocket for this and have the patients and provider sign it. Even have your Medicare patients sign an ABN which is a requirement from CMS to bill a Medicare beneficiary. That way your provider is covered.

Would you bill the insurance for the injection (20610) and the patient for the viscosupplementation? Or is the patient responsible for the entire charge?
