23 year old male seen in the clinic today. recently accepted a position as a loan examiner with the Farm Credit Administration. Because of his hearing loss, he is requesting use of assistive devices. The company requires a written request for accommodations. They are requesting the information for a fuller understanding of the extent of the disability in order to consider effective reasonable accommodations. 
The paperwork will be reviewed. After completion, Jerome will pick up the forms and return them to Phil Shebest, Chief Human Capital Officer. There are other health providers who are also completing forms and Jerome would like to return them all at the same time.
Total time in consultation 30 minutes.
