I have a provider that coded the following, would you code I13.0 AND I15.0? this one has my brain hurting. thanks for any help
Chronic kidney disease stage 4
Renovascular hypertension with goal BP
Chronic diastolic Congestive Heart failure
Diabetes mellitus with diabetic nephropathy
benign essential hypertension
