I am looking for clarification on the use of scribes in the office setting. I do understand that in the hospital setting that in order to bill the service under the physician the NPP must document that they are acting as a scribe and the physician documents that they have reviewed the note for accuracey and state they have personally performed the service, as well as co-sign it. Do these same guidelines apply in the office, or is it sufficient enough that the NPP states that the complete history, exam, assessment and plan were formulated by the physician and they are merely dictating all of this on their behalf, with them both just signing the note without the physician making a statement of review?
 
I have been researching the same thing, and Novitas requires a statement from the physician attesting the review of the chart/note
