Patient is status post colectomy for incarcerated spigelian hernia with strangulation of the colon and now presents for "screening" colonoscopy. Preop diagnosis is: Need for screening colonoscopy. History of strangulated colon. Spgelian hernia with history of strangulated colon, status post colectomy and end colostomy. Patient presents for colonoscopy one day prior to undergoing lap assisted colostomy takedown. Do I use the screening G0121 CPT code or 45378? If 45378 what diagnosis code do I use?
