I am searching for information on a unique situation, but it has to have happened for other facilities:
Patient is 93 and has determined that she elects to have SBRT instead of traditional surgery based on her feelings of risk due to age. She is the one pushing the physician for alternative treatment from the surgery. The Director has asked that I find a code, if there is one, to include with the billing that will indicate this. I do not find one specific, but I am still in my Apprentice-ship. Our Coding department states it would be part of the E/M already performed & notes would just need sent. Patient is on Medicare and we do not want it just be denied standard if we can prevent it. Would it be acceptable to just have the patient sign the waiver and bill with the GY or GZ code on a 99499 for this portion and bill $1.00?
