A patient comes in as a scheduled routine, md finds a rt pvd, which should be coded first. Her ins does not requires copay for routine.

Chief complaint drives primary code selection. I would code with routine diagnosis primary and send to vision insurance. The provider can recommend a followup exam to address medical concerns. The only time I break this rule is when the medical findings are of an urgent nature.
