What guidelines are used to determine acute (23410) and chronic (23412) rotator cuff injuries? If the MD performs an open rotator cuff repair (23410, 23412) and performs an acromioplasty decompressing the subacromial space, is the acromioplasty included in the above two codes? Would you use 23130? Thanks in advance!!
 
I don't think 23130 is included in 23410/23412 but some people beg to differ. 23420 includes acromioplasty.

According to the AAOS, and Medicare CCI edits 23130 is not included in 23410 or 23412.

Here is the difference between codes:
Code 23410 should be reserved for young patients who have an acute episode resulting in a torn rotator cuff and early repair. Code 23412 is more appropriately used for most of the rotator cuff tears that occur in older individuals who have sustained a tear over time, with or without a superimposed acute episode.
