I work for a fast growing multi-specialty physician. I am currently performing a new provider audit on out new urologist. For all of his new patients he documents all pertinent systems, and then he says all other were reviewed and are negative. My question is, when a provider see's a patient for the first time, is it necessary for them to perform a complete ROS or Exam? I was told by another auditor that this can be allowed because this is the first encounter the physician has with the patient. In my experience, I don't not give credit if it is not medically necessary or not related to the CC or reason for the encounter. Does anyone else have any insight or this matter?
