Depends on the severity of the CHF - is it well controlled with Lasix? If so, then I don't think high - maybe moderate?
 
Level 5 risk would include an acute or chronic illness that poses a threat to life or bodily function. So, if the CHF is severe enough, then it's level 5.
 
You also have to look at if the patients pulse generator is at end of life. What is the reasoning for changing the pulse generator? Pt has CHF does not qualify for the change out unless there are more underlying factors and he/she requires an upgrade to a dual pacemaker/icd

Also, don't forget that medical decision making varies on such a simple thing as to if the doctor documented that the risk factors were discussed or not.
