Help!

Our doctor did a right and left heart catheterization, coronary, but the valve was not crossed.

Would i bill code just code 93454 or
would i bill for 93456
or 93458.

in order to bill a LHC the valve must be crossed. According to what you have stated I would code a 93456, coronary angiography with RHC.
