Here is another one...
1. Revision of the reconstructed breast bilaterally with bra role reduction.
2. Bilateral nipple areolar complex reconstruction
3. Excision of nevus, right anterior axillary fold, 1cm with 1cm intermediate
closure.

The Op note indicates a 45mm full thickness skin graft was harvested from the right bra roll area. Can this be billed in additional to the revision or is it included?

Any feedback would be greatly appreciated.

I would code this as 19380-50, 19350-50, Yes also full thickness skin graft(15200-15201) I don't remember how many mm's convert to sq cm's.
Also, I would only code the excision of nevus if there was another incision made to excise that. 11401,12031.
