Revision laminectomy

Which code would be more appropriate 63042 or 63047? The diagnosis is spinal stenosis.

The operative report states there was extensive scar tissue of L4, which was slowly dissected, and the remaining lamina was removed performing a complete laminectomy of L4. The foramen was extremely tight at this juncture, and a medial facetectomy was performed. This was continued laterally with a combination of double action rongeurs and Kerrison rongeurs until the foramina probed freely bilaterally.

Per the AMA 63047 would be correct for a revision laminectomy. But per NASS you would use 63042 if more than 90 days post initial surgery and 63047 if earlier than 90 days post surgery. Confusing I know. I go with 63042 if reexploration and 63047 if revision. :rolleyes:
