I need some help w/this revision case. This is what was done...

L4-5 bilateral laminectomy - revision w/removal of scar tissue
L4-5 bilateral medial facetecomy
L5-S1 bilateral laminectomy & medial facetecomy

The diagnoses are 721.3 & 724.02

I coded 63042 for the revision & 63047 for the L5-S1 laminectomy.

Does that sound correct?
