How do you code for a revision of CPT 27236? The patient has developed an infection and needs the bipolar head replaced. Since the patient has not had a total hip replacement I don't feel any of the normal hip revision codes are warranted.

Are they doing a one stage exchange? How long ago was the fracture and bipolar? If the fracture is healed it would probably turn into 27125 (maybe, depending). They aren't converting to a total? Just doing a head exchange, or just the poly liner?
If it's single stage treatment of infection with arthrotomy, washout, and liner exchange it's 27030. If they are staging it you would look at removal with drug delivery device and then the next stage is the hemi again.
